| 1 - Select Create a Form link at top of page and click Get Started. | |

2 - On the "Choose Template" page select Donations/Surveys/Questionnaires and Donation Form. Click Continue. |
3 - Describe your Organization -- you'll be prompted to answer a few painless questions. |

4 - On the "Review Page" in the Donation to Organization module enter your question text and an optional suggested amount. Click Save. It is now part of your form! See an example. |
