Proceeds from the Scarecrow Shuffle 5K benefit NEDSRA, the North East DuPage Special Recreation Associaton. The Butterfield Park District partners with NEDSRA in service to positively impact individuals with disabilities through diverse recreation opportunites and community services. The benefits of recreation are endless, and, together, we can make a difference to improve the quailty of life for everyone!
FOR EACH PARTICIPANT
1. Scarecrow Shuffle T-Shirt
2. Water Stations: Start, Finish, 1 1/2 mile
3. Free refreshments
4. Raffle prizes(each particpant receives a free raffle ticket)
5. One award presented to the best overall male and female runner. One award presented to 1st-3rd best times in each age group. 1st place in each age group receives a performance t-shirt as part of their award. Participation ribbons available to everyone. Awards are presented after race.
Waiver and Release
The Butterfield Park District is committed to conducting its recreation programs and activities in a safe manner and holds the safety of participants in regard. The District insists that all participants follow safety rules and instructions that are designed to protect the participants'. Participants and parents/guardians of minors registering for the above listed programs/activities must recognize, there is an inherent risk of injury when choosing to participate in recreational activities.
You are solely responsible for determining if you or you minor child/ward are physically fit and/or skilled for the activities. It is always advisable, to consult a physician before undertaking any physical activity, if health issues are involved.
Recreational activities are intended to challenge and engage the physical, mental and emotional resources of each participant. Despite careful and proper preparation instruction, medical advice, conditioning and equipment, there is still a risk of serious injury when participating in any recreational activity. Not all hazards and dangers can be foreseen. There certain risks, dangers, and injuries due to inclement weather, slipping, falling, poor skill level or conditioning, carelessness, horseplay, unsportsmanlike conduct, premises defects, inadequate or defective equipment, inadequate supervision, instruction or officiating, and all other circumstances inherent to indoor and outdoor recreational activities/programs exist. It must be recognized that it is impossible for the Butterfield Park District to guarantee absolute safety.
In signing up and participating in the Butterfield Park District identified activities, you will be expressly assuming the risk and legal liability and waiving and releasing all claims for injuries, damages, or loss which you or you minor child/ward might sustain as a result of participating in any and all activities connected with and associated with said programs/activities (including transportation services/vehicle operation, when provided).
I recognize and acknowledge that there are certain risks of physical injury to participants in these programs/activities, and I voluntarily agree to assume the full risk of any and all injuries, damages or loss, regardless of severity, that my minor child/ward or I may sustain as a result of said participation. I further agree to waive and relinquish all claims I or my minor child/ward may have (or accrue to me or my child/ward) as a result of participating in these programs/activities against the Butterfield Park District, including its officials, agents, volunteers, and employees (herein after collectively referred as Butterfield Park District).
I do hereby fully release and forever discharge the Butterfield Park District from any and all claims for injuries, damages, or loss that my minor child/ward or I may have or which may accrue to me or my minor child/ward and arising out of, connected with, or in any way associated with these programs/activities.
I agree to allow publication of any photos taken at any program, event, or facility of the Butterfield Park District.
I have read and fully understand the above important information, warning of risk, assumption of risk and waiver and release of all claims. If registering via fax my facsimile signature will substitute for and have the same legal effect as an original form signature.
I, the undersigned, agree to allow the individual(s) named hereon to participate in Butterfield Park District activities.
I certify that the participants named is/are physically fit and able to engage in Recreation Services Division activities
In case of emergency, I give my permission for emergency medical treatment.
This form shall be considered valid until canceled or changed in writing by the undersigned parent/guardian/participant.
My signature acknowledges that I have read and understand and agree to the waiver and release on this form.
