The vision of the Eye Bank Association of America (EBAA) is the restoration of sight worldwide. The oldest transplant association in the United States, the EBAA is a nationally recognized accrediting body for eye banks. Since 1961, EBAA member banks have restored sight to over 1,000,000 individuals.

Chip Timing - Chronotrack B-Chip Disposable Chip Tags!
This event features the latest timing technology, the Chronotrack B-Chip! The computer chip comes already attached to your bib #. No need to wait until race day to pick up your chip - it's already on your bib when you go to a Pre-Registration/Packet Pick-Up event!
You must cross the special timing mats at the start and finish lines in order for your time to be recorded.
Bausch + Lomb is pleased to offer complimentary shuttle transportation from three host hotels to the event site and back. Check back soon for pick-up locations and times!
The 5K course will run from Columbus and Balbo south under Columbus Drive and Lake Shore Drive. It turns north along the Harbor Walk to just south of Randolph Street before turning around and heading back south on the bike path that runs alongside the northbound lanes of Lakeshore Drive. The course finishes on the west sidewalk of Columbus Dr - same location as the start line.

Pre-Registration / Packet Pick-Up
Pre-Registration/Packet Pick-Up will take place on Saturday, November 10th, 2012 between 9:00 am and 5:00 pm at the Bausch + Lomb booth (#3126) at the McCormick Place South, Level 3, Exhibit Hall A in conjunction with the AAO/APAO Joint Meeting Conference.

Waiver and Release
I know that running and/or walking a road race is a potentially hazardous activity. I agree I will not enter and run or walk unless I am physically able to do so, have underdone appropriate conditioning, and can do so without jeopardizing my health. I agree to abide by any decision of a race official relative to my ability to safely complete the course. I assume all risks associated with running and walking in this event, including but not limited to: falls, contact with other participants, the effects of the weather, including low temperatures and/or wind chill, traffic and conditions of the road. All such risks being known and appreciated by me. Having read this waiver and knowing these facts and in consideration of this entry, I hereby for myself, heirs, executors, and administrators waive any and all claims I may have for damages against Chicago Running and Special Events Management Inc. dba Special Events Management, the City of Chicago, Chicago Park District, Bausch + Lomb, American Academy of Ophthalmology, Eye Bank Association of America and all sponsors and individuals associated with the event, their representatives and successors, and assignees for any and all injuries suffered by me in connection with this event, including pre and post-race activities. I hereby grant permission to CSEM and its authorized agents to use my name, photographs, videotapes, motion pictures in connection with this event, including recording any other record of my participation in this event for any purpose. There will be a $30.00 fee for all returned checks. Sorry no refunds.
